Job description

As a Life Coach/Qualified Mental Health Professional, you will provide quality Mental Health Skill-Building services to adults through training and support; enable clients to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. Life Coach/Qualified Mental Health Staff will train and reinforce functional skills and appropriate behaviors related to the Client/Individual's mental health and safety, activities of daily living and use of community-based resources. This position reports directly to the Clinical Supervisor.
Life Coach/Qualified Mental Health Staff provide services throughout Hampton Roads, serving urban and rural populations and all surrounding areas.
TASKS and RESPONSIBILITIES:

1) Maintain a caseload, ensuring the provision of Mental Health Skill-Building Services to adults through goal-directed training in the following areas; functional skills and appropriate behaviors related to the individual’s health and safety, activities of daily living, use of community resources (including but limited to supporting clients with locating shelter/housing, financial and food assistance, etc.), assistance with medication management, and monitoring health, nutrition and physical condition.
2) Provide mental health skill building services that are reflective of the attention to the individual’s specific needs.
3) Provide supportive counseling/crisis intervention services to individuals as needed.
4) Develop, maintain, and effectively implement the client's Individualized Service Plan.
5) Develop and review client's quarterly and monthly reports.
6) Develop, implement, and review client's discharge plans.
7) Produce high quality, punctual, and accurate documentation of all casework consistent with Virginia Department of Behavioral Health & Developmental Services licensure and agency policies.
8) Maintain thorough records of all services being provided to clients.
9) Attend and participate in staff meetings and agency trainings.
10) Coordinate care to ensure client's health challenges are addressed.
11) Participate in individual and group supervisions.
12) Maintain ongoing communication with program staff and other treatment providers as necessary.
13) Transport client(s) to appointments and etc.
Note:

Life Coach/Qualified Mental Health Staff typically render services in the community including but not limited to hospitals, group homes & primarily residential settings, etc.
All Life Coach/Qualified Mental Health Staff must hold a Bachelor’s degree in Human services or a related field from an accredited college or university,” or must be a QMHP (Qualified Mental Health Professional-A).
