This position allows for flexibility in scheduling, enabling therapists to choose their hours Monday through Friday. • Conduct thorough evaluations of patients to assess their needs, capabilities, and therapeutic potential in a home care environment.
• Develop and implement individualized treatment plans tailored to each patient's specific recovery goals, utilizing various techniques such as exercise, manual therapy, and therapeutic modalities.
• Utilize advanced techniques and equipment to optimize patient comfort and mobility, focusing on evidence-based practice.
• Document treatment sessions systematically, including tracking progress, and adjusting therapeutic goals based on patient response.
• Educate patients, families, and caregivers on therapeutic procedures, including proper use of assistive devices to enhance recovery.
• Participate in regular team meetings to collaboratively review patient progress and update treatment strategies as needed.
• Stay current with industry advancements by attending workshops, collaborating with peers, and engaging in professional societies.

• Hold a Doctorate in Physical Therapy from an accredited institution.
• Possess a current and valid state license in Physical Therapy.
• Demonstrate effective written and verbal communication skills.
• Exhibit proficiency in computer skills, particularly in documenting patient care and using electronic health records (EHR).
• Show flexibility and adaptability in a changing work environment.

• Prior experience in a home care setting is advantageous but not required.
• Familiarity with orthopedic rehabilitation practices and modalities will be highly regarded.
