Home Physicians Group is seeking a Primary Care Physician practicing in Skilled Nursing, Assisted Living and Independent Living facilities for a Provider run group in Central Florida. We are a large practice with over 25 practitioners that serve the skilled nursing facilities, assisted living facilities and our homebound patients in the comfort of their home in 14 counties. As an Advanced Practice Registered Nurse (APRN), you will play a crucial role in delivering high-quality healthcare services to patients in various settings depending on the location and care to be given. Your primary responsibility will be to assess, diagnose, and manage patient care, ensuring that each individual receives personalized treatment plans tailored to their unique needs. You will collaborate with a multidisciplinary team to enhance patient outcomes and promote health education within the community. Minimum Qualifications:
