St. Francis Hospice (SFH) is growing, and we need an APRN/ Advanced Practice Registered Nurse to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community.

The APRN/ Advanced Practice Registered Nurse is responsible for in-person medical assessments, phone consultations and plan of care management, including appropriate documentation and collaboration with healthcare providers and care partners. Responsibilities include participation in medical staff coverage rotation, clinical quality assurance, pharmacy and utilization review, and performance improvement activities.

An APRN degree with an active and license to practice in Florida. Must hold a Hospice and Palliative Care Certification or become certified within 2 years of accepting position. Preferred experience in the care of terminally and chronically ill patients.
