Job description

Re-Entry Care Coordinator

As Re-Entry Care Coordinator for The Women's Project (TWP), you will deliver a range of hands-on support to assist and coordinate care for justice-involved women from point of detention through successful re-entry in the community, employing a trauma-informed, culturally relevant, person-centered approach. You will be a strong advocate and support participants with building plans toward achieving holistic wellness, economic stability, and strong community support.

Your responsibilities will include:

  • Coordinate care for women from point of detention through successful re-entry in the community, employing a trauma-informed, culturally relevant, person-centered approach.
  • Monitor individual outcomes of participants against goals laid out in individualized service plans.
  • Act as the primary point of contact for a caseload of 10-15 women.
  • Maintain ongoing contact with other agencies involved in individual cases and provide advocacy as needed.
  • Provide clinically-informed individual and group support with a high degree of professionalism and confidentiality.
  • Provide crisis intervention and engage in safety planning on an as needed basis.
  • Connect participants to appropriate agencies for ongoing services, supporting and advocating as necessary.
  • Connect participants to immediate and/or long-term resources for food, clothing, shelter and medical or behavioral healthcare.
  • Connect participants to job training, educational resources, and employment services as needed.
  • Provide court support, including preparation of written court updates and in-person appearances.
  • Visit participants in jails and state prisons, on occasion.
  • Assist in accessing prescribed medication upon release from jail and in the community, as needed or requested.
  • Participate in weekly staff meetings, weekly case reviews, participant events, and professional development trainings.
  • Facilitate communication with legal teams, the Court, government agencies, and other community partners.
  • Maintain thorough and precise case notes and records.
  • Fulfill the Project's and/or funders' documentation requirements, which may include but are not limited to progress notes, termination and transition plan summaries, data collection, and/or other relevant reports.

You're a great fit for this role if you have:

  • A Bachelor's degree in Social Work, Criminal Justice, Public Health or a related field, or equivalent experience.
  • A Master's degree in the above fields or a related credential (e.g., CASAC) preferred, but not required.
  • Strong experience with Microsoft Office Suite or related software.
  • Bilingual in English/Spanish strongly preferred.
  • Flexibility to regularly travel within the five boroughs as well as some out-of-the-area travel on occasion.
  • Experience providing services to those impacted by the criminal legal system, trauma survivors, and/or people with behavioral health needs.
  • Experience working in the New York City social service landscape (preferred).
  • Demonstrated knowledge of:
    • Principles of culturally relevant, trauma-informed care.
    • Criminal legal system (preferred).
    • Motivational interviewing (preferred).
    • Behavioral health planning (preferred).
    • Re-entry services, with a focus on New York City (preferred).
    • Supportive and permanent housing (preferred)
    • Educational and workforce development (preferred)
  • Excellent verbal/written communication and presentation skills and ability to communicate with a variety of audiences.
  • Excellent organizational skills and attention to detail.
  • Excellent analytical, reasoning, and problem-solving skills.
  • Ability to function well in a high-paced (and at times) stressful environment.
  • Ability to maintain the highest level of confidentiality, integrity, and ethical practice.

The Women's Project Summary

The Women's Project (TWP) is a Wildcat community-based program aimed at securing the release of women and gender-nonconforming people held in New York City's jails and preventing the incarceration of women in New York State prisons. TWP staff provide comprehensive care coordination, court support, wellness groups, employment and education assistance, and connections to treatment and housing. TWP is funded by the New York State Division of Criminal Justice Services to provide alternative to detention (ATD) and alternative to incarceration (ATI) services in New York City for 55 women a year.
