Home Health RN

The Home Health RN is responsible and accountable for providing Quality / No Harm, Customer Experience, and Stewardship. Performing interdisciplinary case management and providing appropriate age-specific therapy services.

Primary Accountabilities

  • Performs complete assessment/evaluation including, but not limited to, home safety, pain, nutrition, fall risk and DME needs.
  • Develops/Implements a Plan of Care based on assessment findings and Physician order.
  • Complies with State and Federal Regulations, JCAHO standards in reference to delivery of Home Care Services.
  • Provides Case Management for all disciplines
  • Supervises the Home Health Aide no greater than every 14 days. Supervises the Licensed Practical Nurse (LPN).
  • When needed, assists the Weekend Staff for coverage to maintain continuum of care.
  • Participates in all aspects of Telehealth
  • Documents alterations in the Plan of Care with Treatment Orders sent for signature to MD on the day the change occurs and progress towards goals.
  • Rotates On Call.
  • Evaluates patient outcomes, communicates progress, and changes in condition to physician.
  • Informs patient/family of visit schedule in advance by calling the night before and/or if schedule changes.
  • Ensures appropriate utilization of visits and services for care ordered as noted through proper documentation completed within 2 days of the visit.

Work Experience

  • Graduate of an accredited school of Professional Nursing.
  • Minimum of 1-year acute care experience as an RN. Home Care experience preferred.
  • IV Therapy and phlebotomy experience preferred.
  • Criminal History Screening.
  • Computer skills.
  • Current Florida Drivers License, automobile insurance and reliable transportation.
  • Must hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain.
  • Current licensure in the State of Florida as a Registered Nurse.
