Clinical Liaison Home Health RN PT

As a Care Transitions Coordinator (Clinical Liaison), you will assist patients with the process of navigating post-acute care. Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services.

Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care. Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.

Must be a graduate of an approved school of nursing or therapy. Must be licensed in the state where they currently practice. Must have two (2) years of demonstrated field experience. Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred. A Registered Nurse RN or Physical Therapist PT is required. Three (3) years of field experience is preferred. Previous experience in home health or healthcare sales is preferred. Must possess a valid state driver license. Must maintain automobile liability insurance as required by law. Must maintain dependable transportation in good working condition. Must be able to safely drive an automobile in all types of weather conditions.
