About the job Primary Care Physician
Vivo HealthStaff is recruiting for a Primary Care Physician in North County, San Diego.
The Primary Care Physician is responsible for providing primary care and acute illness evaluation and treatment to patients. They will perform comprehensive assessments on patients, collaborate with the IDT to develop care plans, and actively participate in coordinating 24-hour care delivery. The physician will work collaboratively with other healthcare professionals to ensure effective care coordination. They will also oversee the use of medical specialists and inpatient care for patients. The physician will supervise and provide support to Nurse Practitioners and Physician Assistants.
Essential Functions
? Provide primary care and acute illness evaluation and treatment for patients.
? Perform comprehensive assessments on patients and collaborate with the IDT to develop individualized care plans.
? Collaborate with Community-Based Physicians and the IDT to ensure effective care coordination.
? Plan and coordinate discharges for acute, short-term, and long-term placements in conjunction with the IDT.
? Supervise and support Nurse Practitioners and Physician Assistants.
? Oversee the use of medical specialists and inpatient care for patients.
? Participate in rotating night and weekend call schedules.
? Supervise all care of patients in the nursing home, providing regular visits as necessary.
? Collaborate with the Medical Director and clinical staff to review outcomes, utilization data, and make policy and procedural improvements.
? Protect privacy and maintain confidentiality of all company procedures and information about employees, patients, and families.
? Follow organizational policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
? Maintain professional licensure and certifications within the scope of practice.
Education and Certification
? Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
? MD or DO degree.
? Board Certification in Internal Medicine or Family Practice.
? Additional certifications in Geriatrics, Hospice and Palliative Care, Addiction Medicine, or Pain are desirable.
? Active California physician license without restrictions.
? Current Basic Life Support (BLS) training.
? State CDS Certification and DEA Registration.
? Current driver's license and proof of auto insurance.
Skills & Experience
? Current knowledge of chronic care/geriatric issues and best practices.
? Current knowledge of behavioral health and substance use care.
? Ability to work well in a managed care environment with an interdisciplinary team (IDT).
? Ability to build strong working relationships with other providers and the care team.
? Creative and enthusiastic commitment to improving experience, health, and value for patients.
? Ability to process information accurately and timely.
? Excellent clinical, organizational, and communication skills in settings with frail, elderly patients, their families, and the IDT.
? Working knowledge of relevant regulations preferred.
? Bilingual Spanish-English preferred
