This role serves as a clinical resource and patient advocate, ensuring timely access to prenatal services, patient education, coordination of care among providers, and support of quality maternal and fetal outcomes. The Prenatal Coordinator collaborates with physicians, advanced practice providers, nursing staff, ancillary departments, and community resources to provide comprehensive prenatal care management.**Essential Responsibilities**The following description of responsibilities to be performed is not intended to be all-inclusive. Rather, it focuses on the major tasks that must be accomplished.**Patient Care Coordination*** Coordinate prenatal care services for obstetric patients from initial pregnancy confirmation through delivery.* Monitor patient progression through prenatal care schedules and recommended screenings.* Ensure timely referrals, consultations, diagnostic testing, and follow-up appointments.* Identify high-risk pregnancies and facilitate appropriate provider involvement and specialty referrals.* Assist with care transitions between clinic, hospital, specialists, and community services.**Patient Education*** Provide individualized education regarding: + Prenatal health and wellness + Nutrition and weight management + Prenatal testing and screenings + Pregnancy warning signs + Labor and delivery preparation + Breastfeeding and postpartum care* Reinforce provider care plans and educate patients on treatment recommendations.* Assist patients in accessing educational resources and community support programs.**Clinical Support*** Conduct nursing assessments through in-person, telephone, or electronic communication.* Review patient concerns and triage according to established protocols.* Support medication management and patient adherence to treatment plans.* Monitor laboratory, diagnostic, and screening results and ensure appropriate follow-up.* Document patient interactions and care coordination activities accurately in the electronic health record.**Population Health & Quality Improvement*** Maintain prenatal tracking systems to ensure compliance with recommended prenatal care guidelines.* Monitor quality measures related to maternal and infant health outcomes.* Assist with implementation of evidence-based prenatal care initiatives.* Participate in quality improvement projects focused on prenatal access, patient engagement, and care outcomes.* Generate reports and identify opportunities for process improvement.**Resource Navigation*** Connect patients and families with available resources, including: + Financial assistance programs + WIC services + Social services + Behavioral health resources + Childbirth education programs + Lactation services* Collaborate with social workers, care managers, and community organizations to address social determinants of health.**Team Collaboration*** Serve as a liaison between patients, providers, nursing staff, hospital departments, and community partners.* Participate in multidisciplinary care conferences and team meetings.* Support standardization of prenatal workflows and best practices.* Function as a clinical resource for staff regarding prenatal care coordination processes.**Communication Skills*** Ability to establish and maintain effective working relationships with other staff member.* Ability to exercise good judgment in appraising situations and making decisions.* Ability to work with limited supervision and to accept and profit from constructive criticism.* Ability to identify and maintain agency and client confidential information .**Other Responsibilities*** Perform all other related duties as assigned in a professional and responsive manner* Demonstrate the ability to use relevant equipment* Support, understand, and promote Alomere Health’s mission, vision, values, policies and procedures* Ability to meet the work schedule requirements with flexibility dependent upon the needs of the department* Foster respectful working relationships with professional colleagues, patients, families, and general public regardless of age, gender, lifestyle, culture, beliefs, race, socioeconomic class, or ability## Qualifications**Required Qualifications*** Graduate of an ADN or BSN school or College of Nursing* Registered Nurse in the State of Minnesota* CPR/BLS Certified* Experience in an OB setting**Preferred Qualifications*** ACLS Certification**Knowledge, Skills and Abilities*** Skilled in Microsoft Office and general computer knowledge* Strong verbal and written communication skills**Union Position**No #J-18808-Ljbffr
