What is a Practice Transformation Coach?

A Practice Transformation Coach is a professional who helps healthcare organizations and providers implement changes that improve patient care, workflow, and overall practice efficiency. They guide teams through the adoption of best practices, new technologies, and quality improvement initiatives. Their role often involves training staff, analyzing performance data, and supporting continuous improvement efforts to achieve better health outcomes. Practice Transformation Coaches work closely with clinical and administrative teams to ensure changes are sustainable and aligned with organizational goals.

What are some common challenges a Practice Transformation Coach faces when implementing change initiatives in healthcare settings?

Practice Transformation Coaches often encounter resistance to change from staff who are accustomed to established workflows. • Develop practical approaches, templates and coaching guidance that help Capability Owners establish and mature healthy capability backlogs.
  • Partner with Learning & Development and Change Management to ensure Agile guidance, training, and communications consistently reflect the Capability Operating Model.

Leadership & Capability Coaching (30%)
  • Coach Capability Owners, Agile Leads, and Capability Teams on agile principles and product-centric delivery.
  • Help leaders shift from project-based to outcome- and value-based delivery.

  • Identify organizational impediments and partner with leadership to remove barriers.
  • Promote experimentation and continuous improvement.
