Responsibilities

  • Evaluate and treat patients with physical and functional limitations, developing and implementing individualised treatment plans.
  • Collaborate with the interdisciplinary rehab team, physicians, and nursing staff to support patient recovery goals.
  • Document evaluations, progress notes, and discharge summaries in compliance with facility and regulatory standards.
  • Provide staff education and mentorship to PTAs and rehab aides as needed.

Requirements

  • Active, unencumbered Physical Therapist licence in the applicable state.
  • Minimum 1 year SNF or post-acute rehab experience preferred.
  • Current BLS/CPR certification.
  • Strong documentation skills and experience with EMR/EHR systems.
