The Population Healthย Coordinatorย is responsible forย coordinating patient care activities for patients in need of enhanced care coordination. The Population Healthย Coordinatorย will ensure patients receiveย timelyย andย high qualityย healthcare and support servicesย through the use ofย care protocols, care plans, self-management goals and referrals to the mostย appropriate resources.ย Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke's mission and values.ย
