What You'll Do

As a Home Health Registered Nurse, you will:

  • Build trusting relationships with patients and their families through compassionate, individualized care.
  • Perform comprehensive nursing assessments and identify patient needs.
  • Develop, implement, and revise personalized plans of care.
  • Administer medications, treatments, wound care, and other physician-ordered services.
  • Monitor patient progress and communicate changes in condition to physicians and the healthcare team.
  • Teach patients and caregivers how to manage illnesses, medications, treatments, and safety concerns at home.
  • Coordinate care with physicians, therapists, social workers, aides, and other members of the interdisciplinary team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Provide guidance and supervision to Licensed Practical Nurses (LPNs) and Home Health Aides as appropriate.
  • Participate in on-call coverage as assigned.
  • Ensure care is delivered in accordance with agency policies and all state and federal regulations.

  • Valid driver's license
  • Reliable transportation
