Job description
Position Summary
The Care Management Program Manager position reports to the Director oftheIFS Department and assiststhe Director in developing thegoals,resources,and initiatives of the department,ensuringthe qualityand consistencyofour services, the measurement of ourimpact,andcultivatingthewell-beinganddevelopmentofour team.The ProgramManageroverseesthe CM program by supervising a team of care managers andfacilitatingprofessional development,providing relevant expertise to various populations and age groups, growth of department, andmanagingall related aspects.
The Program Manager also provides direct service by maintaining a small caseload of care management clients, working in an integrated team approach providing care management services and supportive clinical counseling to Individual and Family Services (IFS) clients. The Care Management Program Manager identifies needs of clients and provides care management services (resource navigation, benefit application/enrollment, and financial stability improvement). Services are provided at all JFCS locations and will serve the Greater Philadelphia Area, including remote work. The Program Manager will provide services in client homes, office locations, and remote settings; travel throughout the Greater Philadelphia area is required.
Duties and Responsibilities
Care Management
- Coordinate and provide supportive counseling and care management services for IFS clients through direct services in the client's home, via telehealth, and in the office. Home visits are made to prevent access related issues for the client and made at their request.
- Conduct comprehensive assessments of clients' needs and assesses their goals to develop mutually agreed upon treatment/care plans.
- Work with clients to enable access to resources appropriate to their individual needs (e.g., budgeting, financial issues, and accessing public entitlements).
- Collaborate with community resources (career services, housing) and assists clients in daily life skills to help them function at as high a level as possible.
- Maintain knowledge of a broad variety of community resources and refer clients when appropriate to supporttheir treatment goals.
- Provide advocacy with landlords, community enforcement agencies, family and natural supports, and others as needed.
- Prepare and document accurate and timely records including progress notes, assessment tools, and billing information in compliance with department documentation guidelines and expectations and external regulatory policies.
- Participate in team conferences, staff trainings, and other meetings related to care management issues.
Leadershipand AdministrativeResponsibilities
- Provide an open and engaging environment where employees can communicate freely and work together to improve service to our clients.
- Serve as a resource to the IFSteam.
- Lead the teambyproviding open communication, ongoing coaching and feedback and ensuring performance objectives are met. Recognizes good performance and addresses areas for improvement with individual team members.
- Leadthe teamby means of regularly scheduled individualsupervisionsand case reviews with respect to assessment skills, use of resources and documentation.
- Leadand monitor theteam's work functions, productivity, time management, caseload management and crisis management.
- Ensure team members document accurate and timely records including progress notes, assessment tools, and billing information in compliance with department documentation guidelines and expectations and external regulatory policies.
- Facilitate team meetings/group supervisionto enable opportunitiesforthe teamto share issues and discuss programs and to ensure that team has adequate communication of agency news and policies.
- Provide stretch opportunities so thatemployeeslearn and grow in scope,responsibility,and expertise.
- Ensure newemployeeselection is handled according to level of expertise required, capability andteam"fit."
- Ensure newemployeesareoriented and trained to perform their professional function consistent with program policies and agency's standard of performance.
- Create meaningfulteamevaluations so thatassociatescan understand their strengths and where they need improvement.
- Meet regularly with the Director to plan and coordinate activitiesand assess staff performance.
- Inform the Director of any developing service needs,teamissues, or administrative concernsand provide possible resolution options/recommendationsand plans.
- Attend appropriate administrative,leadership teamand Board Advisory committeemeetingsboth within and outside of JFCS, as assigned.
- Recommend initiatives to enhance theoverall activitiesconsistent with the agency strategic plan.
- Participate actively in strategic discussionswhen appropriate, consideringDepartment scope of service as well as program planning, marketing, implementation, and development issues.
- Work with Director to develop new community collaborations and partnerships, help tomaintainexistingcommunity relationships, both formal and informal, and represents IFSand Agency with community organizations and professional groups.
- Monitor program specific expenses.
- Ensure accurate timekeepingis kept and submitted on a timely basis.
- Perform other duties as assigned by immediateleaderand other management as requested.

- Must be in good standing, maintain any pertinent license(s), and follow guidelines as required by the PA State Board of Social Workers, Marriage and Family Counselors, and Professional Counselors.
- Demonstrate knowledge, understanding, and implementation of practices based on the NASW code of ethics.
- Participate in annual Trauma Informed Care training(s).
Requirements
- Master's degree from an accredited graduate school (MSW required), LSW or LCSW preferred
- 2-4 years professional experience in a related field (social services, counseling, case management) and 2 years supervisory experience (may be concurrent).
- Extensive background, knowledge and skill in counseling and/or care management.
- Proficient use of technology (smart phone/tablet/laptop) and experience using MS Office applications is required.
Additional Requirements
- This positionmay be required to spend some time inall our current and future Greater Philadelphia Areaoffices each week, virtual work will also be required.Travel is a must.
- Some evening or weekend hours may be required based on program need.
- Employee must have access to a car.
- Employee must have a safe driving record on file with the Department of Motor Vehicles in the state in which the employee is licensed to drive. Must have a valid driver's license and proof of automobile liability insurance.
- Employee must pass all necessary criminal and/or background checks.
