Psychiatric Mental Health Nurse Practitioner (PMHNP) – Community Mental Health

Job Details

Salary: $120,000 – $130,000 / year (~$64.00 – $69.00/hr)

Type: Full-time (W2) – 36 hours/week

Location: Salt Lake County, UT (On-site / Community-Based)

Schedule: Monday – Friday, Day shift with rotating on-call backup

Setting: Field Services / Community Mental Health (Up to 500 sq mi area)

About the Role

We are seeking an autonomous PMHNP to serve as the medical cornerstone of a multidisciplinary community mental health team. In this 36-hour/week role, you will provide psychiatric evaluations, medication management, and medical consultation for adults living with serious mental illness (schizophrenia, bipolar I, etc.) directly in the community rather than a traditional clinic.

Core Responsibilities

Clinical Care: Conduct field-based psychiatric evaluations, risk assessments, and ongoing medication management utilizing shared decision-making.

Outreach & Monitoring: Visit clients in homes and shelters; track lab work/metabolic health; coordinate medical referrals and hospital transitions.

Team Collaboration: Lead daily interdisciplinary team meetings, consult on complex psychiatric symptoms, and maintain compliant EMR documentation.

Requirements

Education: MSN or DNP specializing in Psychiatric Mental Health Nursing.

Experience: 2+ years post-grad in community mental health or outpatient psychiatry serving serious mental illness (SPMI). MAT/buprenorphine experience preferred.

Licensure: Utah APRN with prescriptive authority, ANCC PMHNP-BC certification, active DEA, CPR/BLS, and valid driver's license.
