General Statement or Primary Purpose
This is a grant funded, part-time position that assists households approved for housing placement through the Continuum of Care Coordinated Entry referral system in the 5 counties (Beaufort, Bertie, Martin, Pitt and Washington) of Region 12 of the NC Balance of State Continuum of Care. Activities include program enrollment, housing placement assistance, landlord engagement, and supportive services case management.
Duties and Responsibilities or Knowledge, Skills, Abilities
Identify appropriate housing opportunities for eligible clients.
Conduct program eligibility verification and intake.
Assist households as needed during their move-in (e.g. securing volunteer movers, transportation, donation of furnishings, rental assistance funds).
Assist clients with development of a Housing Stabilization Plan.
Review and monitor Housing Stabilization Plan and assist client with implementation as necessary.
Provide linkage to counseling, budget/financial management, job search, and other resources as necessary for success with the Housing Stabilization Plan.
Enter and maintain client data in the Homeless Management Information System (HMIS).
Participate in Housing Support Team meetings as necessary.
Other duties as necessary to support the success of the program and enrolled clients.
Minimum Education and Experience
Four-year degree in community development, social work, or related human services field and 2 years of experience in low income housing programs and/or homeless programs, or any equivalent combination of training and experience which provides the required skills, knowledge and abilities. Consideration will be given to qualified applicants with prior lived experience of homelessness.
Special Requirements
Possess a valid North Carolina driver's license and be willing to travel within Region 12 (Counties of Beaufort, Bertie, Martin, Pitt and Washington) as needed.
Personal transportation may be required when county vehicle is unavailable. Mileage reimbursement available based on the federal mileage rate.
