The LPN assists in the provision of nursing care to the home health patient in their place of residence and collaborates with the patient’s case manager and/or Director of Nursing and other members of the patient care team through the day of discharge.
This is a full-time position in the field for the following areas: Baker, Zachary, Clinton, Ethel, and Slaughter
Responsibilities- Provides home care nursing visits as assigned.
- Instructs patients/caregivers according to the plan of care.
- Make nursing assessments and reports significant changes in the patient’s condition.
- Recommend changes to the plan of care based upon assessment findings.
- Maintains accurate and current drug profiles for patients assigned.
- Documents all patient care on appropriate forms.
- Submits documentation to Pinnacle Home Health within the established time parameters.
- Participates in multidisciplinary case conferences and staff meetings.
- Attends mandatory inoffice, educational inservices.
- Performs other duties as requested/assigned.
Qualifications- Licensed as a practical nurse in Louisiana.
- Minimum of 1 year of experience as a licensed practical nurse required.
- Knowledgeable of nursing assessment skills.
- Competent in the provision of nursing care.
- Phlebotomytrained.
- Maintain current CPR certification.
- 1 year of Home Health experience preferred.
