Job Summary

Pilsen Wellness Center is seeking a dedicated and compassionate Mental Health Professional to participate in the implementation of the individual’s service plan, specifically designed to increase an individual’s skills in independence in daily living, economic self-sufficiency and community integration.

Responsibilities

  • Provides crisis intervention and de-escalation support as needed
  • Conducts ongoing assessment and therapeutic engagement
  • Maintain timely and accurate clinical documentation
  • Assists with data collection and reporting requirements
  • Coordinate care with internal and external resources
  • Adhere to ethical and confidentiality standards

Qualifications

  • Bachelor's degree in Counseling, Social Work, Psychology, or related field required
  • Active Drivers License
  • Bilingual (English/Spanish) preferred
  • Community mental health experience preferred
  • Cultural humility and the ability to build trust with diverse populations
