Job description

We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. As an RN - Home Health Nurse, you will provide high-quality nursing care to patients in the comfort of their own homes. You will play a critical role in helping patients maintain their independence and improve their overall well-being.

Responsibilities:

-Follow plan of care to care for patient in a residential home setting -Follow physician order for care plan

  • Initiate OASIS for all new clients (Skilled, PT, OT, SLP) and do follow up visits as needed
  • Administer medications and treatments per plan of care and MAR -Perform both advanced and basic wound care -Administer IV infusions as ordered and manage PICC and Midline vascular access devices
  • Utilize aseptic technique to prevent infection during procedures -Complete documentation on our EHR system (alora plus) -Ensure all staff (RNs, PTs, OTs, SLPs) on your caseload are completing OASIS in a timely manner and meets the company's quality standards -Provide training and education to new and current staff.
  • Ensure compliance with documentation guidelines
  • Coordinate care with other nurses on case and management as needed
  • Adhere to infection control protocols to maintain a safe environment for patients -conduct audits, assist with training, and other tasks in the field and office as needed

Qualifications:

  • Valid RN license in the state of Virginia or Compact state license
  • Minimum of 1 years of nursing experience, preferably in home health or a related field
  • Proficient in using electronic medical record systems
  • Strong communication and interpersonal skills
  • Ability to work independently and prioritize tasks effectively
