Schedule: MondayโFriday, 8:00 AMโ5:00 PM
Guaranteed Hours: 40 hours per week
On-Call: Rotating every 3rd weekend
License Required: Active Maryland RN License or Compact RN License
Productivity Expectation: 30 points per week
Mileage Reimbursement: $0.43 per mile
Coverage Area: Washington County, MD
Position Overview
We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Washington County. The ideal candidate has a strong home health background, is proficient in HomeCare HomeBase (HCHB) and OASIS documentation, and is passionate about delivering compassionate, patient-centered care. Experience with PICC line management is required, while Wound VAC experience is highly preferred.
Required Qualifications
- Active Maryland RN license or Compact RN license
- Previous Home Health experience
- HomeCare HomeBase (HCHB) experience required
- OASIS documentation experience required
- Experience managing PICC lines required
- Wound VAC experience preferred
- Current BLS certification
- Valid driver's license, reliable transportation, and auto insurance
Responsibilities
- Perform comprehensive patient assessments and develop individualized plans of care.
- Deliver skilled nursing services in accordance with physician orders and home health standards.
- Complete accurate and timely OASIS assessments and clinical documentation using HomeCare HomeBase (HCHB).
- Provide medication administration, patient education, disease management, and health maintenance.
- Assess patient progress, identify changes in condition, and promptly communicate updates to physicians and the interdisciplinary care team.
- Coordinate care with physicians, therapists, case managers, and other healthcare professionals to ensure continuity of care.
- Manage and monitor PICC lines and perform other skilled nursing procedures.
- Provide wound care, including Wound VAC therapy when applicable.
- Ensure medications are ordered, monitored, stored, administered, and documented according to agency policies and physician orders.
- Transcribe physician orders and maintain accurate nursing documentation, including daily, monthly, quarterly, and annual reports as required.
- Maintain infection prevention and control standards while ensuring medical equipment and supplies are properly maintained.
- Supervise and provide clinical guidance to Licensed Practical Nurses (LPNs) and other support staff as needed.
- Coordinate health-related appointments and follow up on physician recommendations.
- Communicate with hospital staff and discharge coordinators to support smooth transitions of care.
- Provide orientation and ongoing education for new employees and annual competency training on health and safety topics.
- Educate caregivers and unlicensed staff on patient-specific care needs, disease prevention, and safety practices.
