Physical Therapist Assistant (PTA) - Winona, MN
Blue Stone Therapy is seeking a Physical Therapist Assistant (PTA) to provide rehabilitation services to adult and geriatric patients at Lake Winona Manor in Winona, MN.
This role focuses on delivering skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapy Assistant works under the supervision of a licensed Physical Therapist to implement treatment plans designed to improve mobility, strength, balance, endurance, pain management, and overall functional independence.
$5,000 sign-on bonus available for eligible full-time candidates.
What You'll Do

  • Provide skilled physical therapy treatment in accordance with established plans of care
  • Implement individualized treatment programs focused on functional and measurable outcomes
  • Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings
  • Monitor patient response to treatment and communicate progress with the supervising Physical Therapist
  • Maintain timely, accurate documentation to support quality care and reimbursement standards
  • Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
