Job description

  • Day shift as-position in the Cardiac Rehab Department

Essential functions of the position that include but are not limited to:

  • Constantly assess for patient safety; identify patients that are at risk for a fall as well as those at higher cardiovascular risk.
  • Accurately take a blood pressure and teach patients normal blood pressure range.
  • Communicate with physicians for referrals and to inform of patient rhythms and blood pressures..
  • Maintain complete Phase II charts for cardiac rehab.
  • Engage patients during their evaluations; setting goals and helping them to make healthier lifestyle choices.
  • Develop and maintain a good working relationship with participants in Phase II & III.
  • Completes the ITP (Individual Treatment Plans) for every Phase II patient every 30 days.
  • Completes discharge paperwork for Phase II patients.
  • Obtain and manage the outcomes data for cardiac rehab Phase II.
  • Scan the Holter monitors daily as they come back.
