Job description

Hospice of the Valley is a national leader in hospice care and has been serving the Phoenix metropolitan area since 1977. Position Profile
The Performance Improvement Specialist provides support to ensure compliance with accrediting and regulatory standards across all service lines. Assesses clinical documentation and identifies areas for performance improvement. Responsibilities include chart reviews, data analysis, research, and processing legal requests.
Responsibilities
  • Conducts quality assessments.
  • Completes medical record reviews and communicates findings in a clear and concise manner.
  • Analyzes and synthesizes quantitative and qualitative information, forming opinions with corresponding recommendations, and shares the results to key stakeholders.
  • Supports Legal Counsel in processing medical record and other requests from regulatory agencies, law firms, and other sources.
  • Conducts special projects related to performance improvement activities to promote efficiency and regulatory compliance.
  • Develops and collaborates in the creation of resources and teaching tools.
  • Maintains current knowledge and skills regarding federal and state regulations and accreditation standards.
  • Seeks feedback regarding individual performance and growth opportunities.
  • Actively obtains new knowledge and skills related to job.
  • Fulfills mandatory educational requirements.
  • Maintains and enhances professional skills.
  • Adheres to high standards of personal and professional conduct.

Minimum Qualifications
  • Bachelor's degree in nursing.
  • Current State of Arizona RN license.
  • Minimum 3+ years' experience in quality and compliance or other legal nursing roles preferred.
  • Minimum of 4+ years of nursing experience.
  • Must have and maintain a valid Arizona driver's license, maintain automobile insurance coverage and have access to an automobile.
  • Basic knowledge of Medicare regulations and state regulations related to hospice.

Preferred Qualifications
  • Master's degree is preferred.
