Job Summary

Supports the development, implementation, and evaluation of programs that advance population health initiatives and Quality-Based Reimbursement objectives. Analyzes population health data to identify opportunities, generate innovative solutions, and drive performance improvement. Partners closely with leadership to translate strategic priorities into actionable operational plans and oversee day-to-day program execution. Leads cross-functional teams and assigned staff, develops and manages project plans and timelines, monitors performance, mitigates risks, manages scope and resources, and ensures effective communication with stakeholders at all levels of the organization.

 

Primary Responsibilities

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job responsibilities performed.

  • Analyze population health, quality, and financial performance data to identify opportunities and develop programs that advance Quality-Based Reimbursement (QBR), care management, and total cost of care objectives.
  • Partner with the Medical Director, clinical leaders, and administrative stakeholders to align initiatives with organizational strategy and ensure effective execution.
  • Lead the planning, implementation, and evaluation of population health and quality improvement projects from initiation through completion.
  • Define project scope, objectives, deliverables, success measures, timelines, resource needs, and acceptance criteria.
  • Develop and manage project plans, schedules, dependencies, risks, and mitigation strategies to ensure projects are completed on time and within scope.
  • Monitor project and program performance, track key metrics and milestones, identify variances, and implement corrective actions as needed.
  • Facilitate cross-functional team collaboration, build stakeholder alignment, and communicate project expectations, progress, and outcomes across all levels of the organization.
  • Develop and deliver reports, presentations, data visualizations, and executive communications that support informed decision-making and organizational improvement efforts.
  • Directly supervise assigned staff, including recruitment, onboarding, performance management, coaching, professional development, employee engagement, and corrective action activities. Foster a collaborative, high-performing team environment that supports departmental and organizational goals.
  • Ensure compliance with organizational policies, quality and safety standards, regulatory requirements, and accreditation standards while contributing to departmental goals and community outreach activities.
  • Perform all other duties as assigned.

Work Experience

Education & Experience - Required

  • Bachelor's degree in Healthcare Administration, Business Administration, Public Health, Nursing, Health Informatics, or a related field.
  • Five (5) years of relevant experience, including experience leading projects, programs, teams, or staff in a healthcare environment.
  • Demonstrated experience leading cross-functional projects, developing and monitoring performance metrics, and translating strategic initiatives into operational plans.
  • Strong analytical, problem-solving, project management, and stakeholder management skills.
  • Experience utilizing data to drive performance improvement and support organizational decision-making.

Education & Experience - Preferred

  • Experience in healthcare, population health, value-based care, and/or Quality-Based Reimbursement program development and oversight.
  • Master's degree in Healthcare Administration, Public Health, Business Administration, Nursing, or a related field.
  • Project Management Professional (PMP) certification.
  • Lean Six Sigma Green Belt or Black Belt certification.
  • Previous leadership or supervisory experience.

Knowledge, Skills, & Abilities

  • Strong knowledge of project and program management principles, methodologies, and tools, including project planning, risk management, resource allocation, issue resolution, stakeholder management, and performance monitoring.
  • Knowledge of healthcare operations, population health, quality improvement, value-based care, and Quality-Based Reimbursement (QBR) models and initiatives.
  • Demonstrated ability to lead multiple complex, concurrent projects and programs through planning, implementation, evaluation, and continuous improvement within a healthcare environment.
  • Strong analytical and critical thinking skills, with the ability to collect, interpret, and analyze data; develop performance metrics; identify root causes; and recommend data-driven solutions.
  • Excellent leadership, interpersonal, communication, facilitation, negotiation, influencing, and conflict resolution skills, with the ability to engage stakeholders at all levels of the organization.
  • Proven ability to build collaborative relationships and lead cross-functional teams across departments and organizational boundaries.
  • Experience facilitating meetings, workshops, and strategic planning sessions involving diverse stakeholders and complex organizational challenges.
  • Strong organizational and time management skills, with the ability to prioritize competing demands, manage multiple initiatives, and meet deadlines in a fast-paced environment.
