Hospice RN – Case Manager | Days | M-F + Call
📍 Missoula,Montana
🗓️ 13-Week Contract
⏰ Typical Schedule: Monday–Friday, 8:00 AM–5:00 PM
📞 On-Call & Weekend Rotation
💼 40 Hours/Week Guaranteed
Requirements:
Active Montana RN License
Minimum 3 years of recent acute care, home health, or hospice experience
Hospice or Home Health experience preferred
Strong clinical skills including wound care, venipuncture, catheterization, and/or parenteral therapy
HCHB (Homecare Homebase) experience strongly preferred
Valid Driver’s License, auto insurance, and reliable transportation required
Responsibilities:
Provide skilled nursing care and case management in patients’ homes
Assess patient conditions and develop individualized plans of care
Coordinate care with physicians and interdisciplinary teams
Provide patient/family education and symptom management
Maintain accurate clinical documentation and OASIS records
Participate in on-call rotation and make home visits as needed
