OBJECTIVE:
To effectively participate in the Rehabilitative Services Team of the Rouse Home in the provision of Per-Diem Physical Therapy Services to individual residents, and to ensure the coordination of resident care between all rehabilitative disciplines and the interdisciplinary care team.
RESPONSIBILITIES
Rehab Team Participation
  1. To support the mission of the Rouse Home/Rouse Estate as a member of the Rehab Services Department.
  2. To support the rehab team's mission and goals.
  3. To attend designated meetings and functions and contributing information as appropriate. Such meeting may include: Medicare meetings, Care Planning, Quality Improvement/CQI, restraint reduction, restorative dining, pressure ulcer, restorative ADL, restorative mobility, or others as appointed.
  4. To ensure coverage of PT services in order to comply with physician orders for all residents.
  5. To ensure proper documentation for billing including MA, MC parts A & B, and cooperate with FMO to successfully bill for covered services.
  6. To work with the Medical Records Department, Nursing and Financial Management Office to ensure appropriate and timely documentation of therapy records and billing practices in accordance with applicable State and Federal law.

Resident Care
  1. To provide interpretation of referrals for Physical Therapy.
  2. To provide initial resident contact and evaluation to establish an initial care plan.
  3. To assist in development, implementation and evaluation of progress, ensuring that residents maintain their highest functional level.
  4. To provide direct resident therapy, as licensed, via screening, evaluation, treatment and monitoring of resident progress. To document direct therapy in an accurate, timely fashion.
  5. To provide treatment or examination of residents receiving PT services at least every 7 resident visits, or 14 calendar days, whichever occurs first.
  6. To work with the Interdisciplinary Care Planning Team, including the residents and families, to provide a comprehensive rehabilitation program for the residents.
  7. To develop or modify care plans as indicated; provide final discharge assessment or establishment of a discharge plan as indicated.
  8. To delegate services to Physical Therapy Assistants as permitted by education and regulations.
  9. Provide appropriate level of direct, on-premises supervision and monitoring of PTAs including documentation of reevaluations, and adjustments to care plans/goals at least every 30 calendar days or as warranted by a resident's change in status.

To provide and document educational services to the residents, staff and families as needed. To be 'on-call' for PTA's certified to receive in-direct supervision. On-call is defined as available by phone and within a 100-mile radius of facility, with the ability to be on-site if warranted. Additional responsibilities may be requested or added as the need arises at the discretion of the Administrator.
Qualifications
  1. Valid PT license in the Commonwealth of Pennsylvania, including maintenance of ongoing continuing education as required.
  2. Experience in long-term care and outpatient services.
  3. Good interpersonal, oral and written communication skills.
  4. Good clinical skills.
  5. Ability to work as part of a self-directed team.
  6. Knowledge of pertinent State and Federal Regulations and Joint Commission Standards pertaining to the provision of Physical Therapy in long term care/outpatient settings.
  7. Completion and maintenance of clearances, back-ground checks and training programs as required by Rouse policies and regulations.

Working Conditions
Works indoors in well-lighted and ventilated halls and rooms. Possibility of cuts or minor burns from instruments and equipment. May be exposed to communicable diseases. Possibility of strains due to moving residents or injury from irrational residents. Must be able to sit, stand, stoop, bend, stretch, lift, push equipment and walk throughout most of the working day. May be subject to electrical and/or mechanical hazards.
INTERFACE:
INTERNAL:
Must be open and available to address concerns of residents/families and create/maintain a calm environment and positive atmosphere of warmth and personal interest. Must maintain positive interface within all levels of the organization, including contracted and on-staff therapy personnel, physicians, and all departments. Must maintain good working rapport with Medical Director, Administrator and members of the Management and Administrative Teams. Must make a positive contribution as a member of the Rehab Services Team. Will be required to provide input relative to the vision and direction of the nursing home; and further, will be required to support final decisions relative to the implementation of policies and procedures.
EXTERNAL:
Must maintain positive interface within the community, with the Department of Health and other regulatory agencies, groups.
