Job description

All Metro Health Care, a Modivcare Personal Care Service, is looking for a Per Diem RN Supervisor. You will have the opportunity to make a positive impact on the lives of our patients by ensuring our caregivers have the skills needed to deliver outstanding care
  • Supervisory / Post-Hospital Visit: $35-$60 per visit (Supervisory visits are typically brief (approximately 15-20 minutes) and include required documentation.
  • Reassessment: $75 per visit
  • Initial Admission: $90 weekday | $125 weekend
  • Office: 350 Northern Blvd Suite 206, Albany, NY 12204 & 1462 Erie Blvd. 2nd Floor, Suite #207 Schenectady, NY 12305
  • Schedule: 2 days a week. 8a-5p

You will...
  • Performs on-site supervisory and teaching visits to evaluate caregivers
  • Provides clinical orientation to caregivers on assigned home care cases.
  • Participates in the training of new nurses and in-services for field staff.
  • Performs initial nursing assessments
  • Counsels patient/caregiver in meeting nursing and related needs.
  • Assigns and coordinates services provided.
  • Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs.
  • Prepares clinical progress notes ensuring any changes in the patient's condition is reported
  • Monitors clinical records for complete and accurate documentation.
  • May include instruction to caregivers and performing skills competency checks.

  • Registered Nurse (RN)
  • 2 years of RN experience
  • 1 year of home care experience required
  • 1 year of direct supervision of Home Health Aides (HHAs) or Personal Care Aides (PCAs) preferred.
