JOB SUMMARY:
Care Coordination is a collaborative process in which the Care Coordinator RN assesses, plans, implements, coordinates, and evaluates options and services to meet an individual's health needs. The Care Coordinator RN facilitates progression of care and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective. Care Coordination improves efforts for patient access to services to improves outcomes and reduce over-utilization of resources. Responsible for managing patient populations with acute, chronic illness and/or multiple co-morbidities, identified as requiring additional transition of care support. The Outpatient Care Coordinator RN will assist in overcoming health system barriers, assisting with social determinants of health,improve patient outcomes while increasing efficiencies throughout the continuum of care within the St. Tammany Health System market.
MINIMUM QUALIFICATIONS:

  • Graduate of accredited school of Nursing, BSN preferred
  • Current LA RN licensure
  • Working knowledge of alternative payment methodology, insurance coverage, post-acute providers as well as community resources
  • Demonstrated ability in planning, organizing, leadership, and communication
  • Strong problem-solving skills
  • Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: Ochsner Health System, American Heart Association, and American Red Cross. The ILCOR course must include a manikin check-off.
  • Two (2) years of Acute Care Hospital, Emergency Department, Case Management, and/or Utilization Management experience preferred

PHYSICAL DEMANDS:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 10 pounds is required. Must be able to work with a moderate level of noise.
