Key Responsibilities

As a Licensed Clinical Social Worker, you will be responsible for:

• Conducting therapy sessions with individuals, couples, and families facing end-of-life issues, grief, and bereavement.
• Implementing crisis intervention strategies when required, providing immediate support to patients and families in distress.
• Completing thorough intake assessments and crafting personalized treatment plans tailored to the unique needs and goals of each patient.
• Developing and maintaining comprehensive progress notes and documentation in accordance with regulatory requirements and best practices.
• Collaborating with medical staff to provide integrated care, ensuring that psychological support is an integral part of the patient’s overall treatment.
• Engaging in ongoing assessment and data collection to evaluate the effectiveness of interventions and adjust treatment plans accordingly.

Patient Population / Client Focus

Your primary focus will be on adult patients receiving hospice care, many of whom are facing complex emotional and psychological issues related to terminal illness. You will also work with their families, providing support and education to help them navigate the challenges of caregiving and mourning. The diverse backgrounds of patients necessitate cultural sensitivity and a tailored approach to each individual’s situation.

Qualifications

To be eligible for this position, applicants must meet the following qualifications:

• Holding an active Pennsylvania state license as a Clinical Social Worker (LCSW).
• A Master’s degree in Social Work or a related field from an accredited institution.
• A minimum of two years of experience in a clinical setting, specifically working with adults and adolescents, preferably within hospice or palliative care.
• Proficient in crisis intervention techniques and management protocols to effectively support patients in immediate need.
• Strong interpersonal, communication, and organizational skills to foster positive relationships with patients, families, and the healthcare team.

Preferred Experience

While not mandatory, candidates with experience in hospice care, grief counseling, or working with multidisciplinary teams will be given preference. Familiarity with electronic health record (EHR) systems and telehealth technology will also be advantageous in this role.

Compensation and Benefits

This full-time role offers a competitive salary ranging from $90,000 to $100,000 per year, commensurate with experience.
