Registered Nurse (RN) - Hospice (Per Diem)

Location: Milwaukee, WI (West Allis area)

Schedule: Days/Nights | Monday-Friday | No weekends

Position Type: Per Diem

Pay Rate : $68/hr

Job Overview:

We are seeking an experienced Registered Nurse (RN) for a per diem hospice/home health role in the Milwaukee area. This field-based position involves providing compassionate end-of-life and skilled nursing care, including patient admissions and case management, across a designated service area.

Key Responsibilities:

Perform hospice and home health admissions and ongoing case management

Deliver patient-centered care in home settings

Coordinate with interdisciplinary teams to ensure quality care

Maintain accurate and timely documentation

Travel within assigned coverage area for patient visits

Coverage Area:

Approx. 30-mile radius from West Allis office

Includes Milwaukee County, Waukesha County, and North Racine County

May require travel up to a 60-mile radius as needed

Requirements:

Active RN license

Hospice and/or home health experience required

Experience with HomeCare HomeBase (HCHB) preferred

Candidates without HCHB may be considered at a reduced rate during training

Strong clinical and communication skills

Additional Details:

Productivity: 24-30 visits per week (based on 40 hours)

No weekend requirement
