Per Diem Home Care Registered Nurse (RN)

This is a unique opportunity for an RN who wants a slower-paced, Per Diem position!

Registered Nurse (RN) โ€“ Home Care | Per Diem | Weekdays Only

No weekends | No holidays | Willing to trainIn-home assessments | Paid travel time | Mileage reimbursed

Schedule: Flexible weekday hours (8:00 AM โ€“ 5:00 PM)

At Home Care Partners is seeking a compassionate and detail-oriented Registered Nurse (RN) to join our per diem home care team. This role is essential in ensuring high-quality patient care and proper supervision of home health aides in the field.

As a Home Care RN, you will support patients and caregivers through in-home assessments, care planning, and routine supervisory visits, while working independently with the support of a strong clinical team.

Home Care Registered Nurse (RN) Responsibilities

  • Conduct initial patient assessments and develop individualized care plans
  • Complete UAS assessments
  • Perform routine supervisory visits for home health aides
  • Orient and educate caregivers on new patient cases and care expectations
  • Complete 6-month recertification assessments
  • Ensure accurate and timely documentation in compliance with NYS and agency standards
  • Communicate effectively with the clinical and administrative team to support quality patient care

Registered Nurse (RN) Requirements

  • Active, unencumbered New York State RN license
  • Home care or community health experience preferred
  • Experience with UAS assessments preferred
  • Strong clinical assessment and documentation skills
  • Reliable transportation and ability to travel within an assigned county
  • Comfortable working independently in the field
