What You'll Do
  • Conduct in-home assessments and create individualized care plans
  • Provide clinical oversight, coaching, and support to caregivers and field nurses
  • Ensure compliance with LHCSA and NYS DOH standards through accurate, timely documentation
  • Collaborate closely with families, providers, and interdisciplinary team members
  • Support staff education and quality improvement initiatives
What You Bring
  • Active, unrestricted RN license in New York (BSN preferred)
  • Minimum 2 years of nursing experience, including at least 1 year in home care (geriatrics preferred)
  • Strong clinical assessment, organizational, and leadership skills
  • Excellent communication and time-management abilities
  • Dependable transportation for local travel within Suffolk County
