Kline Galland Hospice is looking for a compassionate and experienced Hospice Social Worker to join our team on a Per Diem basis. This flexible role is ideal for professionals seeking meaningful work while maintaining a balanced schedule.
Responsibilities:
  • Conduct comprehensive psychosocial assessments for hospice patients.
  • Collaborate with the interdisciplinary team to develop individualized care plans.
  • Provide crisis intervention and therapeutic support to patients and families.
  • Coordinate community resources and referrals.
  • Deliver services in private homes, Adult Family Homes, Assisted Living, Independent Living, and Skilled Nursing Facilities.

  • Master's Degree in Social Work (MSW), Marriage and Family Therapy (MFT), or Mental Health Counseling (MHC) from an accredited institution.
  • Minimum of 2 years of social work experience (1 year in Hospice or Home Health preferred).
  • Strong verbal and written communication skills.
