Role Scope:

The Clinic RN is responsible for delivering direct participant care, supporting clinic-based services, and providing continuity of clinical care for Habitat Health PACE participants. This role supports clinic operational flow, clinical excellence protocols, and occasional home health visits as needed.

The Clinic RN is an integral part of the multidisciplinary care team and works both independently and collaboratively to assess participant needs, support treatment plans, provide education, and ensure accurate clinical documentation.

Core Responsibilities & Expectations for the Role

  • Contribute to a center experience that participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong.
  • Continue to raise the bar. Constructively seek and share feedback, and help us implement changes in order to improve clinical outcomes and experience for participants.
  • Exhibit and honor Habitat's values.
  • Support clinic operational flow and clinical excellence protocols.
  • Perform participant triage, assessments, and vitals during clinic visits, day center visits, and periodic home visits.
  • Administer medications, vaccinations, wound care, injections, and other nursing procedures as assigned.
  • Assist providers with exams and minor procedures.
  • Provide participant education on disease management, medications, and health maintenance.
  • Participate in care team huddles, care coordination activities, and other meetings as assigned.
  • Support population health initiatives, including screenings, follow-ups, and chronic disease management.
  • Document care accurately in the electronic health record (EHR) and ensure documentation complies with clinical, regulatory, and agency standards.
  • Remain flexible to support home health duties as needed, including in-home nursing assessments and skilled nursing visits.
  • Perform related duties as assigned.

Required Qualifications:

  • Current and unrestricted Registered Nurse (RN) license in the state of California.
  • Current BLS (Basic Life Support) certification.
  • Valid driver's license and access to a reliable vehicle for home visits.
  • IV and blood draw certification.
  • Minimum of 2 years of complex clinical nursing experience; ambulatory, home health, or PACE experience preferred.
  • Minimum of 1 year of experience working with a geriatric or senior population.
  • Proficiency with EHR systems and mobile documentation.
  • Strong clinical judgment and critical thinking skills.
  • Strong interpersonal and communication skills.
  • Ability to work independently and prioritize in a dynamic environment.

Preferred Qualifications:

  • Experience working with underserved or vulnerable populations.
  • Bilingual Spanish/English.
  • Prior experience in both clinic and community or home-based care settings.
  • PACE experience and/or ADHC/CBAS experience.
  • Commitment to patient-centered, trauma-informed, and culturally responsive care.
  • Team-oriented with a collaborative mindset.

Working Conditions:

  • Primarily clinic setting with periodic in-home participant visits.
  • Exposure to diverse patient populations and medical conditions.

Compensation:

We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $55 - $68 per hour and is a bonus-eligible position. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.
