Nurse Practitioner / Physician Assistant – Hormone, Peptide & Functional Medicine

Location: Great Neck, NY – In Person
Compensation: $80–$100/hour, based on experience
Schedule: Part-Time, with opportunity to grow
Aesthetic Injectable Experience: A plus, but not required
About the Role
Relive Health Great Neck is seeking an experienced Nurse Practitioner or Physician Assistant with a background in hormone optimization, peptide therapy, medical weight loss, functional medicine, or longevity medicine.

Please Read Before Applying
This position is specifically for providers with hands-on clinical experience in one or more of the following:
  • Hormone Replacement Therapy (HRT)
  • Testosterone Replacement Therapy (TRT)
  • Women's and men's hormone optimization
  • Peptide therapy
  • Medical weight loss / GLP-1 therapies
  • Functional, integrative, or longevity medicine
  • IV therapy and wellness protocols
Candidates with direct experience across several of these areas will be strongly preferred.

If you do not have clinical experience in hormone therapy, peptide therapy, medical weight loss, functional medicine, or a closely related field, this position may not be the right fit.

Relive Health is a modern health and wellness practice focused on personalized and proactive care. Our services include hormone optimization, medical weight loss, peptide therapy, IV therapy, functional and longevity medicine, regenerative treatments, and aesthetics.

Responsibilities
  • Conduct patient consultations, assessments, and follow-up visits
  • Review and interpret comprehensive laboratory testing
  • Develop individualized treatment plans
  • Prescribe and manage HRT/TRT when clinically appropriate
  • Manage medical weight-loss patients, including GLP-1 therapies
  • Evaluate and manage appropriate peptide therapy protocols
  • Monitor progress and adjust treatment plans as needed
  • Educate patients on treatments, expectations, risks, and benefits
  • Maintain accurate and timely EHR documentation
  • Collaborate with RNs, medical assistants, Client Advocates, and other providers
  • Follow all clinical, prescribing, HIPAA, safety, and compliance requirements
Qualifications
  • Active and unrestricted New York NP or PA license
  • Direct clinical experience with HRT/TRT, peptides, GLP-1s/medical weight loss, functional medicine, or longevity medicine
  • Comfortable reviewing bloodwork and developing treatment plans
  • Strong patient communication and education skills
  • Ability to work independently and collaboratively
  • Aesthetic injectable experience is a plus, but not required
  • IV therapy or regenerative medicine experience is a plus
