Job Summary

The Clinical Coordinator Educator is a key leadership role responsible for designing, delivering, and overseeing education and training initiatives across all clinical coordination programs, including CCM, PCM, BHI, CoCM, and other evolving models. This role ensures consistency, quality, and compliance in training efforts by leading onboarding, ongoing education, process implementation, and audit support across the organization. The ideal candidate is a highly organized, proactive educator with strong clinical knowledge, a passion for team development, and a commitment to maintaining regulatory standards and program integrity.

Key Responsibilities

Training & Onboarding 

  • Lead, organize, and facilitate all new hire orientations for Clinical Coordinators across all programs 
  • Develop and deliver comprehensive onboarding plans for new hires, ensuring successful integration and skill-building 
  • Assign, monitor, and sign off on employee proficiency before full deployment into program-specific workflows 
  • Organize and manage remote trainers or preceptors to support distributed or hybrid teams 
  • Provide refresher training or re-education to current staff as needed, based on audit findings, compliance gaps, or program changes 

Program Support & Auditing

  • Support CCM and PCM programs by completing audits and reviewing charts to assist in meeting compliance and CMS deadlines 
  • Assist leadership in disciplinary or corrective action processes related to documentation, performance, or compliance gaps 
  • Perform chart reviews as needed to maintain documentation standards 

Education Leadership & Communication 

  • Plan and facilitate ongoing educational sessions, meetings, and workshops
  • Coordinate guest speakers, subject matter experts, and interdisciplinary presenters
  • Lead training for new documentation templates, EMR tools, and process rollouts 
  • Ensure consistent communication and implementation of changes across all clinical programs 

Hiring & Recruitment 

  • Participate in the interviewing, selection, and onboarding process for new Clinical Coordinators across all programs 
  • Serve as a subject matter expert in evaluating candidate readiness and clinical fit 

Regulatory & Compliance Oversight 

  • Stay informed of CMS, AHCA, and other regulatory changes impacting clinical coordination and documentation 
  • Ensure timely education and communication to the team regarding any policy, billing, or documentation updates 
  • Adapt training materials and workflows in response to changes in industry standards or payer requirements 

Qualifications:

  • Registered Nurse or LPN required; license in good standing 
  • Minimum of 3 years of experience in clinical coordination, post-acute care, or quality assurance roles 
  • Prior experience in education, training, or leadership preferred 
  • Strong understanding of Medicare programs including CCM, PCM, BHI, and CoCM
  • Experience with EMR systems (e.g., PCC, MatrixCare, etc.) 
  • Strong communication, teaching, and documentation skills 
  • Ability to manage multiple training schedules and maintain detailed tracking and reporting
