Matrix Home Care is actively seeking dedicated and compassionate therapy professionals to support our growing patient needs throughout Polk County, Florida. We are currently hiring Physical Therapists (PT), Physical Therapist Assistants (PTA), Occupational Therapists (OT), and Occupational Therapy Assistants (OTA) to provide quality care to patients in the home setting.
This opportunity is ideal for therapy professionals who enjoy working independently in the field, providing one-on-one patient care, and making a meaningful difference in the lives of patients and families.
We are especially looking for providers who can cover Winter Haven, Lake Wales, Davenport, and nearby Polk County communities.

Responsibilities
Provide therapy services to patients in their homes based on the established plan of care.
Complete evaluations, treatments, follow-up visits, and documentation as required based on discipline and assignment.
Collaborate with the clinical team, physicians, and caregivers to support patient goals and outcomes.
Educate patients and families on therapy goals, safety, mobility, exercises, activities of daily living, and home care needs.
Maintain accurate and timely documentation in accordance with company, payer, and regulatory requirements.
Communicate patient updates, changes in condition, and care needs to the appropriate clinical team members.
Requirements
Current Florida license or certification for the applicable discipline: PT, PTA, OT, or OTA.
Prior home health or field-based experience preferred.
Strong clinical, communication, and documentation skills.
Ability to work independently and manage assigned visits in the field.
Reliable transportation and ability to travel throughout the assigned service area.
