Job Overview:

The Patient Support Coordinator serves as the primary liaison between patients, patients’ families, medical providers and vendors. Ensuring patients receive timely, compassionate, and well‑organized support throughout their primary care journey. This role blends administrative expertise with patient advocacy, helping individuals scheduling appointments, insurance processes, explaining treatment plans, patient benefit-related support and knowledge, and effective communication with various clinical teams and external vendors/insurance companies.

Essential Functions:

  • Serve as the first-line resource for patient inquiries via phone, email, and patient portals.
  • Coordinate appointments, follow-ups, and referrals across multiple providers and departments.
  • Support patients in understanding treatment plans, scheduling needs, and next steps in their primary care.
  • Assist with insurance verification and prior authorizations.
  • Maintain accurate and up-to-date patient records in the EMR system.
  • Monitor patient progress and proactively reach out to ensure continuity of care.
  • Escalate clinical concerns to medical care team.
  • Collaborate with nurses, physicians, and administrative staff to streamline patient workflows.
  • Uphold patient confidentiality and compliance with HIPAA and organizational policies.

Qualifications

  • High school diploma or equivalent; associate or bachelor’s degree preferred.
  • Experience in healthcare administration and patient services, preferred
  • Customer support for half a year to one year required.
  • Strong communication and interpersonal skills with a patient-centered mindset.
  • Bilingual English and Cantonese or Mandarin speaking skills, required. Both Cantonese and Mandarin speaking skills, preferred.
  • Proficiency with EMR/EHR systems, Microsoft Teams and other general office software.
  • Ability to multitask, prioritize, and remain calm in a fast-paced environment.
  • Knowledge of insurance processes and medical terminology is a plus.
