Overview
The Med/Surg Telemetry Nurse is responsible for delivering high-quality direct and indirect patient care within the assigned unit. This role requires a comprehensive understanding of departmental and hospital policies, procedures, and standards related to nursing practice. This full-time night shift position involves applying clinical judgment, interpretive skills, and effective communication to support patient care delivery. Frequent interaction, collaboration, and consultation with interdisciplinary healthcare team members are essential to ensure safe, effective, and coordinated patient outcomes.
Responsibilities
Patient Care & Clinical Practice
  • Facilitates, implements, and evaluates the patient Plan of Care to achieve optimal outcomes.
  • Provides timely and appropriate nursing interventions based on patient needs and clinical priorities.
  • Demonstrates strong clinical judgment, critical thinking, and technical competence in delivering patient care.
  • Assesses, analyzes, and communicates patient condition changes and relevant clinical information promptly to the healthcare team.
  • Acts as a patient advocate, ensuring patient needs, preferences, and safety concerns are addressed.

Collaboration & Communication
  • Collaborates effectively with physicians, interdisciplinary team members, patients, and families to promote positive patient outcomes.
  • Communicates clearly and professionally with providers regarding patient status, orders, and responses to treatment.

Patient Safety & Risk Management
  • Identifies patient safety risk factors and initiates appropriate interventions related to fall prevention, skin integrity, restraint use, and other safety concerns.
  • Follows medication safety policies and procedures to ensure safe administration practices.
  • Promptly initiates interventions to address identified safety needs.
  • Completes required Safety Tracking Tools accurately and timely.

Documentation
  • Accurately and promptly documents patient assessments, interventions, responses to care, and provider communications in the medical record.
  • Ensures documentation meets regulatory, legal, and organizational standards.

Delegation & Supervision
  • Appropriately delegates and supervises assigned staff and assumes accountability for all care provided to assigned patients.
  • Monitors team performance and reports performance issues to leadership to ensure safe and effective patient care.

Patient & Family Education
  • Assesses the educational needs of patients and families.
  • Provides appropriate verbal and written education tailored to patient needs and comprehension levels.
  • Documents education provided and patient understanding.

Provider Orders & Care Coordination
  • Reviews, initiates, and implements provider orders accurately and in a timely manner each shift.
  • Ensures continuity and coordination of care throughout the shift.

Prioritization & Time Management
  • Effectively prioritizes responsibilities based on urgency, patient needs, and available resources.
  • Demonstrates strong problem-solving, multitasking, and time-management skills.
  • Functions independently while seeking appropriate resources when necessary.

Professional Accountability & Development
  • Actively seeks opportunities to expand clinical knowledge and skill development.
  • Assists team members and accepts additional duties or assignments as needed.

Qualifications
  • Minimum of 1 year as a nurse
  • ACLS preferred at hire or within 90 days of hire
  • EPIC preferred

Credentials
  • RN - Registered Nurse
  • BLS - Basic Life Saving certification
  • NIH - Stroke Certification

Education
  • Graduate of an accredited nursing school
