POSITION DESCRIPTION

Title: PATIENT CARE COORDINATOR

General: Patient Coordinator works with referral sources, physician’s offices and patients to ensure patients receive their equipment and/or supplies. Patient Coordinators communicate with company billing personnel to provide documents needed for reimbursement.

Qualifications: Graduate of an accredited high school or GED equivalency

Experience/Knowledge: 

1 year experience in DME (Durable Medical Equipment) businesses with a preferable knowledge in CGM (Continuous Glucose Monitoring) devices, Mobility equipment and breathing machines such as PAP devices and NIVs, . Brightree experience is Mandatory.

Good organizational skills. Detail oriented. Willing to function as a cooperative team member. Responsible and cooperative attitude. Clean and neat in personal appearance. Self-starter. Strong verbal communication and listening skills. High quality customer service. Product knowledge. Brightree knowledge. Excellent attendance and dependability. Knowledge of insurance coverage, procedure codes, diagnosis codes and insurance fee schedules. Meet monthly daily and monthly productivity minimum.

Responsibilities and Duties:

  • Patient demographics data entry
  • Emergency Contact information
  • Insurance details, verification and accurate plan assignment with appropriate pay scale
  • Physician NPI, location details, contact numbers, PECOS verification
  • Diagnosis appropriate to service provided
  • Prior authorization requirements and submission
  • CMN data entry, submission and logging
  • Billing information for claims processing
  • Sleep study, face to face notes including all required details for diagnosis
  • Educate customer on insurance benefits, deductibles and co-pays
  • Cleaning up pending orders & denials related to their assigned products.
  • Scheduling

Completes electronic verification of insurance for DME coverage and prior authorization information, including but not limited to:

  • Deductible
  • Effective date
  • Claims address
  • Subscriber ID and group number
  • Secondary insurance for part B coverage
