Position Summary: 

The Admission Intake Coordinator (AIC) supports smooth and timely admissions by processing incoming SNF referrals, verifying insurance eligibility, conducting background checks, obtaining prior authorizations, and entering referral information into the electronic medical record. This role provides ongoing communication and coordination with the Admissions Team and Patient Transition Liaisons, ensuring all required clinical and financial documentation is gathered promptly. The AIC maintains professional, courteous interactions with residents, families, staff, and external partners while upholding strict confidentiality of all resident and personnel information. Additional responsibilities include maintaining accurate departmental records, supporting facility compliance through participation in audits and inspections, attending departmental and quality assurance meetings, identifying opportunities for improvement, and contributing to policy development. The position also promotes a positive, inclusive workplace culture centered on strong customer service and collaborative team support.

Qualifications:

  • High school diploma or equivalent.  

  • Proficient computer skills and excellent time management are required.  

  • Experience with health insurance, Medicare, Medical Assistance, and other health care plans is preferred 

  • Experience in healthcare, skilled nursing facility, or assisted living community
