Healthcare Sales & Community Partnerships

Are you a relationship-driven sales professional with a passion for helping patients heal and thrive at home? We're seeking a motivated Account Executive - Home Health to grow referral partnerships, expand community awareness, and connect patients with high-quality in-home clinical care.

In this role, you'll act as a trusted resource to physicians, hospitals, and community partners-educating them on home health services and driving referral growth through strong relationships and consistent outreach.

What You'll Do

  • Build and maintain strong relationships with physicians, discharge planners, case managers, and community partners
  • Promote home health services and educate referral sources on eligibility, clinical outcomes, and care coordination
  • Drive referral volume and support census growth through targeted sales and marketing activities
  • Plan and implement community education and outreach initiatives
  • Support the development of new programs and service offerings
  • Assist in creating and maintaining annual marketing plans and performance goals
  • Maintain accurate referral, physician, and marketing databases
  • Coordinate business development efforts within medical networks
  • Partner with clinical and operational teams to ensure a smooth referral and admission process
