POSITION SUMMARY: 

The Nurse Coordinator is a Registered Nurse (RN) responsible for delivering in-home patient care, conducting required supervisory visits, and providing field-based clinical support. This role operates in a part-time capacity and supports care delivery, assists with coordination efforts, and helps ensure continuity of care as needed. 

The Nurse Coordinator collaborates with Case Managers, physicians, and the interdisciplinary care team to support patient care in the field. 

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

  • Perform in-home patient visits, including comprehensive health and psychosocial assessments as needed  

  • Monitor patient condition and report changes to the Case Manager and/or physician  

  • Provide patient and caregiver education on disease processes, treatments, and care plans  

  • Complete and submit all clinical documentation accurately and timely  

  • Provide coverage visits when an assigned RN is unavailable to ensure continuity of care 

  • Collaborate with Case Managers and physicians regarding patient status and care needs  

  • Participate in care conferences as requested  

  • Assist with coordination of services and patient needs as directed by the Case Manager  

  • Support discharge planning discussions as requested 

  • Conduct supervisory visits for LPNs, HHAs, and PCAs in accordance with company policy and state regulations. 

  • Document supervisory visits and communicate findings to appropriate team members 

  • Assist with Durable Medical Equipment (DME) orders and follow-up as directed  

  • Support coordination of ancillary services when needed 

  • Remain accessible to patients and team members as needed, including occasional evenings or weekends  

  • Provide flexible support based on patient and operational needs in a PRN/part-time capacity 

  • OTHER DUTIES AND RESPONSIBILITIES: 

    This position requires traveling between locations in your personal vehicle 

    SUPERVISORY RESPONSIBILITY: 

    This position does not directly supervises any other position directly but may perform supervisory visits as directed by a Clinical Manager and/or Case Manager. 

    Qualifications

    PREFERRED QUALIFICATIONS:  

    • Active, unrestricted Registered Nurse (RN) license in the applicable state(s) of practice  

    • Current CPR certification  

    • Prior experience in home health or a similar clinical setting preferred  

    • Demonstrated clinical assessment, critical thinking, and communication skills  

    • Working knowledge of applicable home health regulations, standards, and best practices  

    • Ability to work independently while exercising sound clinical judgment in a field-based environment  

    • Ability to meet all credentialing and onboarding requirements, including successful completion of a criminal background check and any other pre-employment screenings as required by law or company policy
