Overview
Job Summary
Coordinates and provides care utilizing the critical thinking framework. Blends caring, compassion, knowledge and integrity to provide safe, timely, and quality care while facilitating movement of patients throughout all of Baptist facilities. Patient placement nurses are responsible for ensuring that patients are placed in the most appropriate level of care based on their medical condition and needs. This involves coordinating transfers between hospital facilities and liaising with healthcare providers to ensure that patients receive the appropriate level of care. Makes appropriate bed assignments for admissions, ED, intra-hospital and entity transfers. Performs other duties as assigned.
Responsibilities
  • Assess bed availability for all areas of the hospital.
  • Proficient in electronic bed management systems.
  • Participates in activities designed to improve health care delivery.
  • Provides clinical leadership.
  • Supports lifelong learning.
  • Completes assigned goals.
  • Coordinating patient admissions and transfers: The patient placement specialist coordinates with healthcare providers, patients, and families to ensure that patients are admitted to the appropriate units or transferred to other facilities as needed.
  • Assessing patient needs: The specialist assesses patients' medical conditions, diagnoses, and treatment plans to determine the best unit and level of care for them.
  • Collaborating with healthcare providers: The specialist collaborates with physicians, nurses, and other healthcare providers to ensure that patients receive the appropriate care and treatment.
  • Ensuring proper documentation: The specialist ensures that all necessary patient information is accurately documented and communicated to the appropriate healthcare providers.
  • Managing patient flow: The specialist manages patient flow throughout the hospital, ensuring that patients are moved efficiently through the system to prevent delays or bottlenecks.
  • Facilitating communication: The specialist serves as a liaison between patients, families, and healthcare providers to facilitate effective communication and ensure that patients' needs are met.
  • Maintaining compliance: The specialist ensures that all hospital policies, procedures, and regulations are followed, including those related to patient privacy and safety
  • Following the escalation process which involves speaking with Administration and House Supervisors
  • Facilitates transfer back process between our entities
  • Monitor and coordinate care for emergent/urgent conditions (STEMI) through Pulsara.
  • Enters orders timely for Strokes and STEMI's
  • Assist facilities with problem-solving with patient movement/orders

Minimum Required
  • RN and 1 years in acute care.

  • Knowledge of Baptist hospitals and specialty units. EPIC experience.
