The Master’s Touch Home Care, Inc. is seeking a dedicated and compassionate Part-Time/PRN Physical Therapist to join our team. We are committed to providing high-quality, personalized care in the comfort of our clients’ homes, and we are looking for a professional who shares our passion for service and excellence.
Position Summary
The Physical Therapist will provide skilled occupational therapy services to home care patients in accordance with physician orders and the plan of care. The therapist will evaluate, plan, and implement treatment programs to help clients restore mobility, improve strength, and maintain independence in their daily lives.
Responsibilities
• Conduct patient assessments and develop individualized care plans.
• Deliver physical therapy treatments in the patient’s home.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on exercises, safety, and mobility strategies.
• Document care accurately and timely in compliance with agency and regulatory requirements.
• Collaborate with interdisciplinary team members to ensure continuity of care.
