Position Overview:

Qualifications:

  • Education: Master's degree in Nursing from an accredited program.
  • Licensure: Must have an active New York State Nurse Practitioner license. Must be Board Certified or Board Eligible.
  • Experience: Must have experience in the specialty for at least 1 year ( DERM-cosmetic does not count)
  • Technical Skills: Proficient in diagnosing and treating common dermatologic conditions such as pressure ulcers, skin infections, rashes, psoriasis, eczema, skin cancers, and fungal infections. Skilled in performing skin biopsies and other minor dermatologic procedures. Proficiency in eClinicalWorks.
  • Soft Skills: Strong clinical judgment, excellent communication and interpersonal skills, empathy, patience, and the ability to work collaboratively with nursing home staff, families, and other healthcare providers.

Key Responsibilities:

  • Provide comprehensive dermatologic evaluations for nursing home residents, including full-body skin examinations.
  • Diagnose and manage acute and chronic skin conditions commonly seen in the elderly, such as pressure injuries (bedsores), cellulitis, fungal infections, scabies, eczema, psoriasis, actinic keratosis, and skin cancers.
  • Perform minor dermatologic procedures including skin biopsies, cryotherapy, curettage, and excisions as appropriate.
  • Prescribe and manage topical and systemic dermatologic medications, monitoring for efficacy and adverse reactions.
  • Develop and implement individualized treatment plans in collaboration with the interdisciplinary team (nurses, wound care specialists, primary care providers, and attending physicians).
  • Educate nursing home staff on proper skin care, pressure injury prevention, infection control measures, and early recognition of dermatologic conditions.
  • Educate residents and families on skin health, treatment options, and preventive measures.
  • Maintain accurate, thorough, and timely documentation in electronic medical records.
  • Collaborate with the multi-specialty practice and referring physicians to ensure coordinated care.
  • Participate in quality improvement initiatives related to skin health and wound care within the nursing home setting.
