WORK SCHEDULE: 1-3 days/week
FLSA CLASSIFICATION: Part-Time, Non-Exempt
COMPENSATION: The compensation range for this position is $35,000-$55,000, with the opportunity to earn additional income through a variable compensation (commission-based) structure, offering higher earning potential based on performance.

We are a multi-specialty healthcare organization dedicated to improving care for residents in skilled nursing facilities. Our clinicians provide essential medical and behavioral health services that support facility care teams and ensure patients receive consistent, high-quality care.
Founded in 2018 and headquartered in Chicago, we partner with skilled nursing facilities across the Midwest and East Coast. Our providers deliver integrated clinical services including internal medicine, psychiatry, psychotherapy, and podiatry, helping facilities meet the complex medical and behavioral health needs of their residents.
Position Summary:
Vitae Health is seeking both full-time and part-time Psychotherapists to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' overall well-being and adjustment to medical and life changes.
Key Responsibilities:
  • Conduct comprehensive psychosocial assessments for residents upon admission and as needed
  • Develop, implement, and monitor individualized treatment plans
  • Provide individual, group, and/or family therapy sessions
  • Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
  • Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns
  • Collaborate with physicians, nursing staff, social workers, and rehabilitation teams to coordinate care
  • Participate in care plan meetings and interdisciplinary team discussions
  • Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations
  • Provide crisis intervention and behavioral management strategies when needed
  • Educate staff and families on mental health conditions and effective communication strategies
  • Ensure compliance with HIPAA and ethical standards of practice

Requirements
Qualifications
  • Master's degree in Psychology, Counseling, Social Work, or related field
  • Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist)
  • 2+ years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings
  • Strong clinical assessment, diagnostic, and treatment planning skills
  • Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders)
  • Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)
  • Ability to work collaboratively within an interdisciplinary team
  • Excellent communication and interpersonal skills with residents, families, and staff
  • Strong organizational, time management, and documentation skills
  • Familiarity with electronic health records (EHR) systems preferred
