Shift: Part-Time, 3 Days a week

We are seeking a dedicated and licensed Social Worker, responsible for identifying, assessing, and addressing the psychosocial, mental, and emotional needs of our residents and ensuring their rights are upheld.

Responsibilities: Psychosocial Care & Advocacy

As the Social Worker, you will be the resident's primary advocate and emotional support liaison. Your core responsibilities include:

  • Assessment: Help to accurately identify resident’s psychosocial, mental, and emotional needs through regular visitation and observation.

  • Documentation: Complete Social History and Psychosocial assessment of residents promptly upon admission and routinely thereafter, ensuring compliance with federal and state regulations.

  • Care Planning: Integrate assessment findings into the interdisciplinary care planning process (MDS), contributing to a holistic plan that addresses the resident's unique needs and preferences.

  • Advocacy: Work closely with the interdisciplinary team to promote and protect resident’s rights, dignity, and self-determination throughout their stay.

  • Protection: Prevent and address resident abuse, neglect, and exploitation as mandated by law and professional licensure.

  • Discharge Planning: Coordinate discharge planning services, ensuring smooth transitions for residents back to the community or to other levels of care, and linking them with necessary post-discharge resources.

  • Support: Provide emotional support and counseling to residents and families coping with illness, disability, and long-term care placement.

  • Other duties as assigned by the Administrator.

Qualifications:

We are looking for a dedicated professional who is licensed and passionate about long-term care.

  • Education: Bachelor's Degree in Social Work (BSW) is Required.

  • License: Must possess an active, unencumbered LSW (Licensed Social Worker) or CSW (Certified Social Worker) license in the state of Wisconsin.

Experience: Experience in a Long Term Care (LTC) or Skilled Nursing Facility (SNF) is highly preferred and strongly recommended.

Skills:

  • Demonstrated ability to work independently while also functioning as a collaborative part of the interdisciplinary group.

  • Proficiency in computer knowledge for documentation (EHR/MDS).

  • Exceptional ability to build and maintain rapport with residents, family members, and peers.
