Job description

Job Title: Registered Nurse (RN) , Home Health — Jonesboro, GA

Overview:
As a Home Health RN serving Loganville and surrounding areas, you’ll provide professional, compassionate nursing care in patients’ homes. Your responsibilities include assessment, care planning, medication administration, and coordination with physicians, caregivers, and other healthcare providers.

Key Responsibilities

  • Conduct initial, routine, and recertification assessments in-home to evaluate patient needs.
  • Develop and update individualized plan of care based on physician orders, patient assessment, and family.
  • Administer medications, treatments, and therapies (e.g., wound care, IVs, trach care) in accordance with the prescribed plan.
  • Monitor vital signs, symptoms, and overall clinical status, and document in medical records (including OASIS assessments where applicable).
  • Educate patients and family/caregivers on disease management, medication administration, safety practices, and equipment usage.
  • Coordinate care with physicians, therapists, aides, and agency team; organize regular case conferences and status reporting.
  • Supervise or delegate tasks to Home Health Aides or Unlicensed Assistive Personnel as needed, maintaining oversight and accountability.

Qualifications & Requirements

  • Current Georgia RN license required; must adhere to Nurse Licensure Compact regulations if applicable.
  • Experience with children with special needs.
  • Minimum one year of recent clinical experience, preferably in home health, hospice, hospital, or skilled nursing facility; OASIS assessment experience preferred.
  • CPR certification is mandatory; additional certifications (e.g., wound care, pediatric home care).
  • Valid driver’s license and reliable transportation to visit patients at home.
