Job description

Occupational Therapist at Creekside Post-Acute, you will play a pivotal role in enhancing the quality of life for patients recovering from various physical and neurological conditions. Your primary objective will be to assess, plan, and implement individualized therapeutic interventions that promote independence and functional ability in daily activities. You will work closely with a multidisciplinary team to develop rehabilitation programs tailored to each patient's unique needs, including those with cerebral palsy, stroke, traumatic brain injury, and orthopedic challenges. This role demands a compassionate and patient-centered approach to support geriatric rehabilitation and sensory integration therapies. Ultimately, your efforts will contribute to patients' improved mobility, self-care skills, and overall well-being, facilitating their successful reintegration into their communities.

Minimum Qualifications:

  • Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
  • Valid state licensure or certification to practice as an Occupational Therapist.
  • Demonstrated experience in rehabilitation settings, including geriatric and neurological patient care.
  • Strong knowledge of occupational therapy principles, sensory integration, and therapeutic modalities.
  • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.

Preferred Qualifications:

  • Experience working in a post-acute care or long-term rehabilitation facility.
  • Certification in specialized areas such as stroke rehabilitation, traumatic brain injury, or pediatric occupational therapy.
  • Familiarity with electronic medical records (EMR) systems and documentation software.
  • Additional training or coursework in physical therapy techniques and orthopedic rehabilitation.
  • Ability to lead or participate in clinical education and staff training initiatives.

Responsibilities:

  • Conduct comprehensive patient evaluations to determine functional abilities and rehabilitation needs.
  • Develop and implement personalized occupational therapy treatment plans targeting physical, cognitive, and sensory challenges.
  • Collaborate with physical therapists, speech therapists, physicians, and nursing staff to coordinate holistic patient care.
  • Monitor patient progress and adjust therapy interventions accordingly to maximize recovery outcomes.
  • Educate patients and their families on therapeutic techniques and adaptive strategies to support ongoing rehabilitation.
  • Document patient assessments, treatment plans, and progress notes in compliance with regulatory standards.
  • Participate in interdisciplinary team meetings to discuss patient goals and discharge planning.
  • Stay current with advances in occupational therapy practices and rehabilitation technologies.

The required skills in occupational therapy, physical therapy, and rehabilitation are essential for assessing patient needs and designing effective treatment plans that address both physical and cognitive impairments. Expertise in geriatric rehabilitation and cerebral palsy enables the therapist to tailor interventions that improve mobility and daily functioning for diverse patient populations. Knowledge of sensory integration techniques is applied to help patients with neurological conditions regain sensory processing abilities. Skills in stroke and traumatic brain injury rehabilitation are critical for facilitating neuroplasticity and functional recovery. Additionally, orthopedic rehabilitation skills support the restoration of musculoskeletal function, ensuring comprehensive care that promotes patient independence and quality of life.
